This one is made by Propoganda and sells for $10 (currently sold out at Juvie Shop ) and also can serve a dual purpose as a door stop.
I think it would most likely just be ignored! Similar items, (just not as ascetically pleasing), have been around for years in the form of animals...etc.

I have a pink heart-shaped one left over from when I had a roommate that would just walk in all the time. If my boyfriend was staying over, I'd lock the door anyway, but I usually put the sign up when I just wanted her to buzz off. It's true, though, that it rarely worked... she'd just walk in anyway without knocking.

AKB2003 - My rude-as-hell neighbors refused to leave me alone, so I finally hung a Do Not Disturb sign on my front door and let it be known among the neighborhood gossips that I would call security if I was bothered again, by any uninvited party, for any reason other than a serious emergency.
It's been up for six blissfully peaceful months. You might consider giving it a try.

I collect real ones from hotels I've stayed at. Some are more visually pleasing than others, but all remind me of a fun place. I keep one on our bedroom from our wedding night - it's the best of the bunch. But they serve no practical purpose.

I handpainted a kitschy one for my Dad a few years ago, but instead of "Do Not Disturb" it reads "No Cat, sleeping" and "Okay, Cat", to let my cat know when he's allowed to go in Dad's room. It was a joke gift but it's been on Dad's doorknob ever since.
I think they can be a handy thing to have. I prefer to have a whiteboard on my bedroom door, I'll leave a note if I don't want to be disturbed.

Only really would be successful in use in a roommates situation if both could respect it or for a guest room(s). More than likely in a family situation, people aren't going to respect it.

The one pictured here is u g l y. But we have a one on our door that has some vintage wizard of oz pictures on it or something. It's just for looks, doesn't stand out too much.

I have the same one in orange... actually bought it for "someone" to hang it on the office door, since so many people just dropped in without making appointments.
Didn't really work well as door stopper or people stopper.
