If anyone ever stops through Ann Arbor, Michigan, we have fairy doors installed inside and outside many local businesses. Its a fun afternoon "fairy hunt" for kids and adults.

We have the top left one in our backyard and it looks so adorable. My 6 yr old son's excited face when he discovered it was just priceless. It inspired us to make some outdoor furnishings for the fairies nearby. An amazing book to go with the door is Fairy Houses Everywhere by Tracy Kane - it is just wow and very inspirational!
One note though - these doors don't come with anything to attach them to the tree, so ours was just propped against the trunk....not ideal when a toddler comes and knocks it down when they try to open it.
Those are awesome. As for making it stick to the tree, you could use little nails to "frame" it. Don't go through the door, put catch the edge on one side and then catch the edge on the other.
A small nail wouldn't hurt the tree, especially if it's the finishing type.
