Handmade bedding on Etsy seems to fall into five categories: the ultra-feminine; the simple and rustic; the mod and funky; the graphic silkscreened. I didn't get into quilts here, because there are just so many, but stuck instead to handmade duvet, sheet, and pillowcase sets. Have a look:

Vintage linens on eBay are another good non-mass-produced source.

While we're on the subject, I need a new set of sheets for under $100. No sateen or sky high thread counts required, they just need to be comfy!

I have to point out that my friends have a little studio called Variegated that makes amazing duvets and throw pillows. They're currently having a steep sample sale on Etsy. The stuff is not cheap, but they use incredibly beautiful, high-end fabrics, develop gorgeous original screenprints, and their tailoring is impeccable:
