I'd like to know this too
I know I've seen this lamp many times before. A little online searching and you should find it. Lampsplus.com has about a thousand lamps, if they don't have this one they would definitely have one similar.
I'm not sure, but I've seen many in that style floating around at flea markets in the LA area. Probably your best bet is to do some hunting there, CL and eBay.
There is one at the Broadway Antiques Market in Chicago, I was just there today.
