So it's pretty simply a video feed from a digital camera strapped to his back is being fed to a small LCD screen on his front. He writes:

This was my Halloween getup for '06. By using a travel DVD player strapped to my stomach, with video coming from a digital camera strapped to my back, it creates the illusion that I have a very large hole in my stomach.

From then on, cutting holes in your sweatshirt and applying the fake blood is up to you!
