Pretty nifty! Would there be any kind of electrical hazard with this?
It's a pretty great little contraption, though in order to have it hanging and hiding the wire all the time, you'd have to have your charger plugged in all the time, even when you aren't charging your phone. If you don't want to be drawing phantom power, then you've got the problem of what to do with the charger and caddy when it's not plugged into the wall...

I have one of these that I bought at Bed Bath and Beyond, but it doesn't have the cup part, so the phone kinda slides off. I like this much better!

As long as the plastic is clean, dirt conducts electricity.
You can just get a single outlet with a switch on it, plug it in to the wall outlet, plug the charger into it, and switch it off when not in use. Most hardware stores carry these: http://www.lowes.com/lowes/lkn?action=productDetail&productId=197460-334-BP4404W-L&lpage=none
I would use a taller bottle and alter the shape of the back (part with the hole in it) so that it can be mounted under the wall plate. Then you don't have to hang it from the charging adapter/plug.
