Whitelines makes a notebook and app they call Link. You write your notes in the physical notebook and then use the app to snap a pic. From there you can email, upload to Evernote or Dropbox. They offer a few PDF samples to download, print and try. Someone could probably use those sample pages and design their own templates and create their own notebooks and note pads...
