It looks like there might be a false panel under the vanity that hides the plumbing. Must be where the towels are, also.

I was in a boutique hotel in Spain that had this style of bathroom. I would wake up in the middle of the night when my husband would go to the bathroom because the bright lights would shine through the frosted glass.

Any bathroom with double sinks is not "small".
