Good Questions: Identify & Source this Mirror?

Hi Apartment Therapy, Can you tell me where I can find the mirror pictured here from the May 2009 issue of Cookie? I swear I have seen this mirror before and I can't locate it for the life of me. It's just one of those pieces that grabs you and I must have it! Thanks for your help! Jennifer

I feel like I've seen it, or something very similar, at Ballard Designs (www.ballarddesigns.com), but I can't find it on their website now.

I found a slightly smaller version of this in white at Home Sense (the Canadian version of Home Goods) for $25! I painted it yellow. Pic here:

That mirror was featured in Domino magazine awhile back and they said you could get it at High Fashion Home in Houston TX. http://www.highfashionhome.com/flower-mirror-black.html Hope that helps!
