Q: I bought this chair and need to have cushions made for it. I will buy the fabric on moodfabrics.com but need to know: Who should I send the chair to in NYC to have the custom cushions made? Don't want to break the band.

If all you are looking for is the foam, I just ordered a replacement cushion from this place and was happy with it.

I'd love to know how to repair or replace straps such as the ones pictured here. I have a chair that is built just like that that needs to be fixed.
Hobby lobby (or a craft store) sells 2-4" foam that you can cut to size, and someone with some basic sewing skills can sew cushions with piping around the edges. I know because I just completed one myself....good luck!
I have this exact chair! Mine still had the original white vinyl seats. I did buy some extra cushions for it though. The cushions on some outdoor chairs are usually sized just right. Since it's end of summer you could probably get some for a steal.:)
I'm glad you aren't going to break up the band!
Sorry,no advice on the chair.
Years ago I was in your shoes (sometimes it is really hard to find certain things in NYC) and finally made some cushions myself, with predictable results, unfortunately. My suggestion would be to look online to see if you can find some pre-made, properly sized cushions. There are a lot of end-of-the-season sales going on right now for cushions, so you might be able to find a deal if you act fast.
Canal foam and futon (I think they're now Economy Foam & Futon Center will sell you the foam, a tailor can do the rest if they won't)
Replacement straps: http://www.fagasstraps.com/ I ordered these for a koford larsen chair a while back and they fit perfectly.
I don't have an answer but LOVE the chair!
craigslist upholstery/tailoring services in your area. Ask for quotes for when you provide your own materials. I think most should be able to handle rectangular cushions.
Try Galeano's Reupholstery Corporation
4810 Skillman Avenue, Long Island City, NY

Try Dixie Foam

I had cushions made at Mod Restoration in Brooklyn. I bit pricey but very good work. www.modrestoration.com
For the old school experience go find Spence in Brooklyn on Myrtle Avenue near Hall Street. He is a great local guy who does solid upholstery work and the price is right. He's been there forever.
The shop looks abandoned but don't let that fool you.
Sorry, don't have his phone. Google Maps link (shop is left of the Lotto awning) http://g.co/maps/ur2bf
Yourself... If that is a rectangle, just get home depot to cut a wooden bottom that fits, put on some foam, put fabric on that and staple gun it to the bottom...
I have a similar chair and would like to get new straps as well. The rubber straps help, but will wear in the long run. Anyone have suggestions for canvas or nylon straps?

dragonphly already provided a source upthread.
Go to Economy Foam and buy the right size, then go around the corner to Stanton Tailor Shop. Pablo will make you cushions for a mind-bogglingly low price.
