This actually seems like a fairly simple endeavor for those with the proper tools. Anne and her husband took a hacksaw to a candelabra, separating one of its arms from the base (sounds gruesome, but not too difficult!). Then they connected the arm to their plate by drilling a hole through both and using a screw, two washers, and a cap nut to finish it off.

Anne has posted some helpful photos and more detailed instructions on the Design Daily blog at Point Click Home. Jamali Gardens is her go-to source for affordable materials for this project; you could also certainly find some workable candelabras and plates at flea markets and thrift stores.

Now this is a wonderful and original DIY project and the result doesn't look cheap!
Very nice, I just think it looks the best when the "mirror" doesn't look like a transformed tray at first glance. Then again on the other hand you could want to enhance the "transformed object" aspect.

Beautiful. But as the survivor of not one but two house fires[one caused by an unextinguished cigarette and the other caused by a candle], I strongly encourage wiring these for candelabra bulbs and skipping the candle. Not to mention that candle smoke and parafin will stain your ceiling over time.

Martha Stewart has something similar to this. You take the end part of a towel bar and turn it on it's side. mount that part to the wall. the candle goes in the hole. then hang a silver tray behind it. you can do silver, like in this post, or use a white plate with a white candle holder.
