Just incase leaving the tooth under your pillow isn't "official" enough, your child can fill out this letterpress certificate of deposit and leave his/her tooth in a silkscreened cloth deposit bag.
Here are the official "instructions" over at the Office of the Tooth Fairy:
Simply place tooth in the attached envelope and insert your completed certificate in the deposit bag. Place the full bag under your pillow. The tooth fairy will collect the Certificate of Record and place your compensation in the deposit bag.

Just precious - lovely design and great idea for the first lost tooth.
I picked up some of these for new moms for Christmas and they loved them. Highly recommend. I might buy 2 for myself and hang on to them until I have kids.

When my daughter lost her first, I told her that the fairy really appreciates a letter explaining how the tooth was lost. She bought it and I now have the sweetest keepsake ever to give her when she's grown.
Oh! Love this! But my boys only have six teeth each. I reckon we're a long way off from losing them. Oh well.
